Random event if King Skarl thinks you told him the worst joke ever at Grumpy Old King.
Tooth Faerie
Random event when you get the Tooth Faerie (which itself is a random event).
Wheel of Misfortune
Have the Wheel of Misfortune turn one of your items into a pile of sludge.
If you have no items in your inventory, you will not get the avatar.
Petpet Laboratory
Have your petpet zapped into a Pile of Soot at the Petpet Laboratory.
Refreshers
To get these, you just have to refresh at a certain page. Boring! Some of them like Cellblock and Dice-A-Roo are games, though, and you get them while playing the game.
Battle Faerie
Win a 1-player battle in the Battledome and refresh the "And the winner is..." page.
This avatar occasionally changes to this "Fight" avatar.

Have a fire petpet attached to your Fire Blumaroo for 70 days or more, and view the Blumaoo's pet lookup.
This will not work with a petpet that could get you a different avatar (e.g., Snowbunny), but it will work if that petpet is not yet old enough to give you the avatar.

Beat challengers in the battledome to get these. They should have one for losing to Punchbag Bob (I've done it).

Defeat the Space Faerie in the Battledome.
